Voluntary Critical Illness Protection
Available only in Oregon, Idaho and Utah

Many employees, or their family members, will experience a critical illness in their lifetime. When a critical illness strikes, basic monthly expenses such as mortgage payments, groceries, utilities, child care expenses and out of pocket medical expenses can quickly deplete savings intended for children’s education or retirement. Because Regence Life and Health’s Group Voluntary Critical Illness Protection pays a lump sum benefit on the first diagnosis of a critical illness, we believe you will want to consider adding it to the benefits you offer to your employees.

CRITICAL ILLNESSES:

  • Cancer
  • Heart Attack
  • Stroke
  • End Stage Renal Disease
  • Quadriplegia
  • Major Organ Transplant (On UNOS list)
  • Carcinoma in Situ (cancer that is confined to the site of origin and has not spread) - 10% of face amount to maximum of $5,000

  • COVERED SURGICAL PROCEDURES:

  • Coronary Artery Bypass Surgery: 25% of face amount
  • Balloon Angioplasty, Stent, or Laser Relief Obstruction Procedure: 10% of face amount

  • FLEXIBILITY:

    Because employees’ financial needs vary, Regence Life and Health’s Group Voluntary Critical Illness Protection allows each employee to select the amount of protection that is right for them and for their family members.

    OPTION TO COVER FAMILY MEMBERS:

    When the employee enrolls, they may elect to enroll their spouse and children.

    Employee elects amount of coverage in $5,000 increments ($5,000 minimum) to a maximum of $100,000. If coverage is elected for the spouse, the amount does not need to match the employee amount. The employee can elect to enroll all children for either $5,000 or $10,000.


    NON-INTEGRATED BENEFIT: This benefit is not reduced by and does not coordinate with any medical or disability benefit for which the employee or family member may be entitled.

    PRE-EXISTING CONDITION LIMITATION: 12 mo/12 mo in Oregon (6 mo/12 mo in Idaho),(6 mo/6 mo in Utah)

    ELIGIBILITY: Employees and spouses under age 65 may apply. Unmarried children must be under age 23 (under age 26 in Utah) and either reside with the employee or be dependent on the employee for more than 50% of their support.

    All applicants are subject to underwriting approval.

    30 DAY WAITING PERIOD: First diagnosis of a critical illness must be made after the initial 30 days of coverage. (N/A in Idaho)

    BENEFIT PAYMENT: Payment is made to the employee in one lump sum at the first diagnosis of the critical illness.

    DIRECT BILL PLAN: If an enrolled employee loses eligibility for coverage for any reason, the employee and any covered family members may continue coverage through The Direct Bill Plan.

    MINIMUM PARTICIPATION REQUIREMENT: Two employees, subject to health statement approval.
